Any participant under the age of 18 must have a waiver signed by HIS/HER LEGAL GUARDIAN in the presence of a Doc*36 employee or signature MUST BE NOTARIZED!

Doc*36 CAN NOT accept any hand written notes or allow a friend/neighbor or  family member to sign for your child/children. If you are 18 or older please be prepared to show proper ID.

THERE ARE NO EXCEPTIONS!
